The PBSCI Facilities Card Access Request Form is offline and not anticipated to return to service. We suspect that this form has been offline since Saturday, 9/27/25, if you submitted an access request form on or after that date, please contact us with the information below. The Facilities Team is working on creating an alternative form or application process. We will share an update once a new process is in place.
In the interim, please contact pbscikeys@ucsc.edu with the following information to request new electronic/card access:

- PI or Supervisor:

* Your ID card number is the six-digit number located on the back of your ID card, above the magnetic stripe.
